1. What is brain herniation?
A life-threatening condition where a part of the brain is squeezed out of its normal position due to increased pressure within the skull, into a region that it does not normally occupy. It causes mechanical and ischemic damage and leads to severe neurological damage and even death.
2. Name any 4 causes?
(a) Traumatic brain injury – increased ICP
(b) Brain tumors
(c) Stroke: Bleeding or swelling in the brain after a stroke
(d) Infections: meningoencephalitis, Brain abscesses
(e) Hydrocephalus
3. Identify the different types of brain herniation, mentioned in the Fig Any 4
(a) Supratentorial - Uncal transtentorial herniation, Central tentorial herniation, Subfalcine herniation, transcalvarial herniation
(b) Infratentorial - Upward transtentorial herniation (“reverse coning”), Foraminal or tonsillar herniation (“coning”)
4. Describe the clinical features?
A sudden, intense headache, Altered consciousness, dilated or unequal pupils. Paralysis, particularly on one side of the body. Papilledema, Seizures, posturing (decorticate, decerebrate)
Breathing problems: Cheyne-Stokes breathing, Biot’s, Respiratory arrest Irregular heart rate or blood pressure
