1. What is the diagnosis?

Bullous impetigo - Large, flaccid bullae filled with clear or yellow fluid, associate with systemic symptoms - Fever, malaise, lymphadenopathy, and occasionally diarrhea

2. What is the causative organism?

- Staphylococcus aureus, especially strains producing exfoliative toxins (ETA, ETB, ETD).
- Mechanism: Toxins cleave desmoglein-1 (Dsg1), disrupting cell adhesion in the epidermis, leading to blister formation

3. What is the treatment?

- Topical antibiotics: Mupirocin, fusidic acid for localized cases.
- Oral antibiotics: Cephalexin, clindamycin, or TMP-SMX for extensive or systemic involvement.
- Avoid: Penicillin (ineffective against S. aureus)
